The JCC seeks to fill two full-time case manager roles, dedicated solely to the CRED program, to provide case management and encourage participant engagement and retention.
Roles and Responsibilities:
- Support the broader team during outreach and intake
- Devise and administer service needs assessment, identifying supportive services or other needs a participant needs to be successful in the program, for each participant.
- Work with Mental Health Professional and referral agencies as needed.
- Provide case management and encourage participant engagement and retention
- Leverage knowledge of benefits and entitlements and ability to navigate complex systems and agencies to assist participants in obtaining resources to successfully complete training
- Connect participants to JCC departments and external agencies that may provide tools and resources to assist participants to successfully complete training.
- Work with participants to identify and work through barriers to program completion
- Work with participants to track progress made towards goals
- Escalate cases as appropriate to program mental health professional
- Work seamlessly with broader CRED team to provide holistic support to all participants
- All other tasks as administered by supervisor.
Minimum Qualifications:
- Successful experience supporting and demonstrated understanding of social service needs of the CRED target population: under/unemployed adults 18-40, involved in the criminal justice system or impacted by community violence
- Demonstrated experience supporting case loads
- Demonstrated knowledge of social services/benefits and entitlements
- Case management experience
- Demonstrated cultural competence
- Excellent communication skills
- Bachelor's degree in social work or a related field and experience working with City systems such as public assistance, child welfare, education, and housing; or
- Associate’s degree and experience working with City systems such as public assistance, child welfare, education, and housing; or
- At least three (3) years successful experience in providing case management/counseling and working with City systems such as public assistance, child welfare, education, and housing.
SALARY
This is a full-time, in-person position, $50,000-$55,000 Annually.
