Principal Responsibilities:
Position Summary:
The case manager will be responsible for assessing, monitoring, and organizing services for clients seeking support from the Marks JCH Social Services department. They will be directly involved in providing resources and assistance; helping clients navigate health services, employment and other benefits available to them. You will be working with social workers, job specialists, domestic violence counselors and other related specialists, offering clients a full-service support system. The case manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help vulnerable adults. You will often be the first point of contact for many of the clients, establishing trust and determining need will be essential.
Responsibilities:
- Provide case management, information, and case assistance services, service coordination, referrals, and advocacy related to benefits, entitlements, and housing.
- Provide support and resources for clients; inclusive but not limited to government resources, internal services including cash assistance and community resources.
- Research and refer clients to internal and external resources such as childcare, job development, legal and psychological support and Medicaid/Medicare.
- Advocating and raising awareness on behalf of clients and the needs of the community and local services.
- Assist with program outreach and communication.
- Develop timely responses and interventions for clients', and conduct ongoing evaluations and follow-up.
- Conduct regular record keeping. Ensure maintenance of accurate client records in the database, oversee data collection and entry; complete statistical reporting as needed.

This is a full-time non-exempt position located in Brooklyn, NY. Candidates can expect an annual salary of $50,250 and a comprehensive benefits package including medical and other insurance access, generous time off, and a pension plan. - Possesses strong spoken and written communication skills; excellent interpersonal skills
- Excellent time management skills; able to synthesize large amounts of information
- Comfortable in a fast-paced, multifaceted environment
- Demonstrated strong commitment client empowerment and working with a diverse population.
- Bachelor's degree in social work, psychology, healthcare or pubic administration and related fields required
- Computer skills using MS Office, Word, Excel, Access, importing/exporting data to/from applications
- Bilingual Russian/English or Cantonese/English are strongly preferred
